Assumption of Risk
I acknowledge that this activity involves known and unanticipated risks, including but not limited to sprains, fractures, head and neck injuries from flips or awkward landings, collisions with other jumpers, and contact with frames, springs, or padding. I understand that such risks cannot be eliminated without jeopardizing the essential qualities of the activity, and I expressly agree and promise to accept and assume all of the risks existing in this activity. My participation is purely voluntary, and I elect to participate in spite of the risks.
Jumping Rules
I agree to jump within my ability, to occupy only one trampoline at a time, to avoid double or uninspected flips, and to never land on my head or neck. I understand that court monitors may remove any participant who ignores the rules, without refund.
Grip Socks
I understand that approved grip socks are required on all trampoline surfaces at all times.
Release and Waiver of Liability
In consideration of being permitted to participate in trampoline and adventure attractions on the premises, I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify, and hold harmless the business, its officers, employees, and agents (the "Released Parties") from any and all claims, demands, losses, and liability arising out of or related to any injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the Released Parties or otherwise, to the fullest extent permitted by law.
Medical Authorization
I certify that I am physically fit and have no medical condition that would prevent my safe participation. I authorize the business to secure emergency medical treatment on my behalf if needed, and I agree to be financially responsible for any such treatment.
Severability and Governing Law
I agree that this agreement is intended to be as broad and inclusive as permitted by the laws of the state in which the activity takes place, and that if any portion is held invalid, the remainder shall continue in full legal force and effect.
